Discharge Criteria
When a tenant’s needs match one or more of the discharge criteria, a family conference will be held with family, tenant (if appropriate), Home Care Coordinator and Facility Manager. Plans will be made to begin the discharge or transfer process.
Moving
- all moving arrangements will be made with the Facility Manager
- timing of move arranged to cause least disruption to existing tenants, security and safety.
- Greendale Estate appreciates as much notice as possible, but 1 month paid notice is mandatory.
Absence from Greendale Estate
Supportive Housing tenants may come and go as they please, but only in the care of approved escorts. Escorts are required to notify Greendale Estate staff of departure time and estimated return.
No credits are given for meals missed due to absence from the facility, with the exception of planned absences of more than 3 days when proper notice of leaving and returning dates is given. Credits will only apply to the charge for services.
